Lost or Stolen Passports

If your passport has been lost or stolen, please report it to the American Embassy and Burkina Faso National Police as soon as possible.  We recommend that you obtain a police report from the local police and bring the report with you to your appointment at the Embassy.

All adults (age 16 and older) replacing a lost or stolen passport must appear in person and present the following items:

  • Original proof of citizenship (e.g. U.S. birth certificate, Consular Report of Birth Abroad, Certificate of Naturalization, or Certificate of Citizenship); and
  • One valid form of photo identification (e.g. foreign passport, driver’s license, military ID, etc.); and
  • Two passport photographs; and
  • Completed forms DS-11 and DS-64; and
  • Fee of $165 or equivalent in CFA

This service is available by appointment only.  Appointments must be made online at the State Department’s Appointment System webpage. Payments are made at the time of appointment using cash or credit card.  We do not accept checks.

All minors (under age 16) replacing a lost or stolen passport must appear in person with both parents.  If one parent cannot be present, please review the Bureau of Consular Affairs’ Passports for Minors Under 16 webpage for guidance.

All minor passport applications must include:
